Create a Journal Entry
Create journal entries to document patient visits using either free-text notes, structured Advanced Journal Templates, or a combination of both. Advanced templates allow you to capture information using dropdown menus, radio buttons, and drawings, making documentation more structured while still keeping the flexibility of traditional clinical notes.
⚠️ Published journal entries are visible to the patient. If you’re not ready to share the entry yet, you can save it as a draft. Draft entries aren’t visible to the patient but will be automatically published after 2 weeks if not published manually.

Step-by-step
1. Go to the patient’s Patient360 and click Journal.

2. Click Create to start a new journal entry.

3. You can document the journal entry using:
Plain text
Plain text template, or
An Advanced Journal Template for a more interactive and dynamic entry, including drop-down menu, radio buttons and drawings.
You can also combine all three within the same entry.

4. If you want to use Advanced Forms, click Add New under Advanced Forms.
Note: To use Advanced Journal Templates, they must first be submitted to Adoreal so they can be uploaded and made available in your system. Contact support to submit them.

5. Choose the Advanced Journal Template you want to use from the list.

6. Click Use template to add it to the journal entry

7. Complete the template using the available fields, such as dropdown menus, radio buttons and drawings

8. Once you are ready, click "Submit". You can always edit your answers as long as the entry isn't published

9. Click the X to return to the journal entry

10. You can add free-text notes alongside the Advanced Journal Template if needed.

11. You can also use a free-text template if you have uploaded one.

12. Then add a title for the entry.

13. If needed, add attachments, images, products, implants, or medications to complete the documentation.

14. You can choose Save as draft and return later to complete and publish the entry.
Note: Draft journal entries are not visible to the patient. If a draft isn’t published manually, it will be automatically published after 2 weeks and then become visible to the patient.

15. When everything is ready, click Complete and Publish.

16. After publishing, the journal entry appears in the patient’s Journals.

17. If an Advanced Journal Template was used, you will see it attached to the entry.

18. It will also appears under Patient Files

19. Click the file to view the completed form.

20. You can review all responses and download the completed form as a PDF if needed
