View and Manage Dynamic Forms and Consent
The Forms & Consents section in Patient360 gives you a complete overview of all forms that have been sent to the patient, including their status, completion details, and activity history. From here, you can review the patient’s submitted information, check whether signatures are in place, and make sure all required documentation is ready before the appointment. This helps ensure a smooth workflow and accurate patient records.

Step-by-step
1. In the patient's Patient360, click the arrow next to Patient Files to open the Forms and Consents section.

2. Here you’ll see every form and consent that has been sent to the patient, along with its status, last triggered date, and if it has been signed.

3. Click any form to open it and review the patient’s answers or signatures.

4. Click the three dots to see more information, such as past activity and which appointment the form is linked to.

5. You can have the patient sign a pending form in the appointment by clicking on it. The consent will open up, together with the option to sign.
