Add Consents and Dynamic Forms to a Service
You can attach Dynamic Forms & Consents to each service and treatment provider so patients automatically receive the correct information, consent forms, or questionnaires based on the appointment type they’re booked for.
Forms are sent out directly after an appointment is booked and and are linked to the specific appointment type, such as consultation, surgery or post-op, making it easy to manage what patients need to complete before their visit.
⚠️ To add a consent or dynamic form, you first need to have added it as a template: Create a New Form or Consent Template
Step-by-Step
1. Open Settings from the top navigation bar.

2. Click "Team Management"

3. Find and click the provider whose service you want to add a form to

4. Inside the provider profile, click the Services tab

5. Find the service you want to edit and click View Details

6. In the Overview tab, you’ll see every form linked to the service, across all appointment types.

7. Here, you can see which appointment type each form is connected to and whether it's a legacy form or a new, dynamic form

8. To add a new form or consent, click "Add Form"

9. Select the template you want to use

10. Select one or more appointment types where the form should be sent out

11. Click "Save"

12. Your new form now appears in the Service Forms list. Click the form to edit it or update its appointment type assignments.

13. You can also open any appointment type in the left menu to view forms linked specifically to that appointment type.

14. Here, you'll see which forms are linked to that specific appointment type

15. Remember to add the necessary forms or consents to all services and treatment providers who need them.
