Create a Consent Form Template

This article walks you through how to create a consent form template and connect it to a specific service. By doing this, the correct consent will automatically be sent to the patient when they book that service. This helps ensure legal compliance and a smooth digital experience for both staff and patients.

Step-by-Step

1. Click the gear icon in the top right corner.

2. Click on "Consent templates"

3. Click on "Add Template"

4. Enter the name of your consent template and write or paste the full consent text in the editor and click on "Save"

5. Back in the settings, go to “Location Management.”

6. Click on "View Details"

7. Go to "Services"

8. Click “View Details” on the relevant service.

9. Click "Add Consent"

10. Choose your newly created consent template from the dropdown list.

11. Click "Save version"

12. Click “save” to confirm and activate the consent for this service. It will now be automatically sent to patients who book this treatment

13. ⚠️ Tip: If you want the same consent form (e.g. for Breast Augmentation) to be used across all clinic locations, you’ll need to repeat this process for each location individually.
