Create a Consent Form Template

This article walks you through how to create a consent form template. This will then allow you to set up consents that will be sent out to patients when they book a surgery. Setting up consent forms helps ensure legal compliance and a smooth digital experience for both staff and patients.

⚠️Once you've created a Consent Form Template, you'll need to Add Consent Forms to a Service

Step-by-Step

1. Click the gear icon in the top right corner.

2. Click on "Consent templates"

3. Click on "Add Template"

4. Enter the name of your consent template and write or paste the full consent text in the editor and click on "Save"

5. The Consent template is now added and can be reused for all services. To implement it, you'll need to add it to the specific service and treatment provider.
